    General Nature of Work: Under the care of an RN, Use & practice assessment data collections, participate in planning. Execute interventions in accordance with the Plan of Care, Communicate to ensure patient/family is educated to the plan of Care & planned treatments. Maintain an environment of safety for patients, self & others & support patient safety initiatives. Assess patient’s condition, including biophysical, psychosocial, age specific needs & safety needs. Document patient’s status according to unit specific & nursing division policies & procedures. Collaborate with other health care team members in formulating plan; Implement the nursing plan of care on assigned patients. Adhere to the American Nurses Association Code of Ethics. Recognize, support, and utilize evidence based nursing practices. Follow all Physicians Orders as Given. Administer medications as ordered. Provide home care visits and assume on-going primary responsibility for an assigned caseload of patients within a geographical area. Responsible for conducting home assessments & evaluations of patients nursing & healthcare needs. Coordinates activities with other health team members to ensure optimal patient care & communication of pertinent findings.
     
    Qualifications:  Minimum Education Requirements/Certification/Licensure for Licensed Practical Nurse Job. Diploma from an accredited school of Nursing. BLS certification through the American Heart Association.
     
    Other Desired Qualifications Applicable to this Position:  Preference will be given to applicants who, in addition to meeting the minimum qualifications, possess one or more of the following:  supervisory experience of other staff, experience providing direct care and training to persons with disabilities.

    ( x )          A pre-employment PPD Tuberculin Skin Test will be required at the applicant’s expense prior to appointment to position.
    ( x )          An FBI Criminal History Record Check is required prior to appointment to position.

    ( x )          Males between l8 and 26 years of age will be required to present proof of having registered with the Selective
                    Services System as required by federal law or of being exempt from such registration.
